We can assist in taking Energy Share of Wyoming applications when applications are open. When funding is available Utility Assistance, Rental Assistance, Food by Choice Pantry. On Occasion we also assist with: Diapers, Wipes, Hygiene, Toiletries, Adult Diapers, Bed Pads.

These services are designed to keep people in the least restrictive environment for as long as possible. CommuniCare is a Community Information Exchange® (CIE), a network of service providers who share client information and coordinate care with each other from a centralized technology platform. Partners of CommuniCare can access a CommuniCare Navigator via referral through the CIE platform. Our goal is to ensure that immediate needs met so that they can begin to build towards stability, financial independence, and achieving their goals.

Housing First offers case management and employment assistance. Recover Wyoming also offers Telephone Recovery Support (TRS) Program.

Our Intensive Case Management Program supports a limited number of qualified individuals and/or families who meet 125% of the federal poverty guidelines. Approved participants of Project Hope will enter a commitment. Each obligation will receive intensive case management, including but not limited to achieving and/or maintaining housing; enhancing life skills; addressing individual needs; building social skills and community relations, and developing goals to achieve an improved quality of life.

Funded by state and federal grants along with assistance from Crook County Commissioners, Towns of Sundance, Hulett, and Moorcroft. We offer a Nutrition Program, Senior Companion Assistance, Medical Alert, Respite Care, Personal Care, Homemaking Services, AARP Driver Safety Classes, Social Activities, Blood Pressure & Toenail Clinics, Transportation and more!

Cheyenne Regional Medical Center’s Transition Across the Community Team (TACT) program is a no-cost, patient-centered program designed to improve quality of life, reduce re-hospitalization and contain costs for patients who are 18 years of age and older and considered high risk. If you’re experiencing an episode of illness, you may receive care across multiple settings—often resulting in incomplete or confusing information.

We provide court supervised substance abuse counseling, supervised probation and case management services to qualified participants in Park County.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.
