Rent & Utility Assistance

Children's Learning Center provides therapy and special education services for children in Sublette County birth to five years as well as preschool programs for all children 2-5 years. The Children's Learning Center also provides developmental, vision, hearing screenings for all area children (at no charge to the families) whose families inquire.

Children are screened in all developmental areas. If a concern is identified children may receive evaluations from professionals in any, or all, developmental areas. With parent approval the team prepares an individualized education plan or individualized family service plan. Early intervention services are provided free of charge. Children up to 3 years receive services in the home or childcare. Children ages 3 to 5 receive services in a preschool environment.

Youth and family support to help with intensive care coordination school, counselors, DFS and probation for families with children who have a behavioral health condition, who need more support, or are risk of out of home placement.

Assist those in need with accessing health care, mental health care, and other social services.

One Health Providers recommend patients complete an annual wellness exam to stay up-to-date on their primary care. During Wellness visits, Providers discuss Preventive Care with patients. Recommended Preventive screenings, immunizations, and other interventions vary per patient based on age and gender. Patients should check with their insurance company to learn what services are covered in a Wellness Visit.
